GASTROENTEROLOGY AND ENDOTHERAPY

Combined registration applies when 1 physician and 1 nurse register together.

Participants applying in the category "Trainees" (under the age of 32) are requested to join a copy of their identity card or passport mentioning their birth date confirming they are under the age of 32. If such document is not provided before the beginning of the Workshop, the fee difference will be requested on-site to the participant.

ESGE Individual Member registration : ESGE individual members receive a 20% registration fee reduction (for early registrations before April 20, 2011). While registering online, you will be requested to provide your ESGE individual member identification number (4 digits), so that the system can check your membership and apply automatically the 20% discount. This benefit is only valid for registration in the 3 days of the Workshop and may not be claimed at the same time with other potential reduction (group registration for instance).

The fees cover : Participation in the scientific sessions including the satellite symposia, sandwiches-lunches, coffee-breaks, welcome party (if registration to the welcome party has been made before June 8, 2011).

PAYMENT
Fees are in Euro (European Currency Unit). Payments in other currencies will not be accepted. No registration will be processed without full payment.

- By credit card
Payment is accepted by VISA, EUROCARD or MASTERCARD only. The cardholder's name and signature, card number and expiry date as well as the CVC code on the card (i.e. the last 3 digits on the back of the card) must be clearly indicated on the registration form.

- By bank transfer

In order to avoid misunderstandings, participants are kindly requested to indicate clearly their name on bank transfer order forms. The Workshop secretariat will not be responsible for tracking transfers deposited without appropriate identification. Bank charges are the responsibility of the sender.

PAYMENTS BY CHEQUE ARE NOT ACCEPTED.

REGISTRATION DEADLINE
From and after JUNE 9, 2011, pre-registration will NOT be accepted. Please do not send any registration form or payment. You will have the possibility to register on site, taking into consideration that you will not benefit from our very fast procedure offered to pre-registered participants and that it will not be possible to register to the social programme events.

VISAS AND LETTERS OF INVITATION - NEW !
Visas are not required for visitors from most of European Countries. For all other countries, visas are issued on a reciprocal basis. It is advisable to contact the local Belgian Embassy or Consulate. For all participants, we have developped a new tool in our registration system. As soon as you are registered to the Workshop (even if you do not have paid online), a registration confirmation e-mail is sent automatically, with, as attachement, an invitation letter that can be printed and used for visa purpose.

CANCELLATION POLICY
A refund of registration fees already paid, less 20% for administrative expenses, will be made to any participant who becomes unable to attend the Workshop, if notice in writing is received by the Course Coordinator prior to June 1st, 2011. After June 1st, 2011, there will be no refund. All refunds will be handled after the Workshop.

NAME SUBSTITUTION
Name substitution received before May 5th, 2011 will be free of charge. After May 5th, 2011, a handling processing fee of Euro 80 per substitution will be charged.

Name substitution or replacement occuring after June 8th, 2011 will be invoiced as a full registration.

GROUPS REGISTRATION
For groups (more than 10 participants), the bio-medical companies or travel agencies in charge should contact the Course Coordinator before sending the registration forms or on-line registration and before payment.

CONFIRMATION OF REGISTRATION
The participants who register through this website will receive at the end of the procedure a confirmation that they are duly registered to the Workshop, as well as a "confirmation of their order". Formal confirmation of registration with necessary details for arrival and stay in Brussels will be sent in the course of May-June 2011 by e-mail and by post. The organisers can not be held responsible if the "sponsor" or "travel agent" in charge of your registration does not provide your personal e-mail address for further contacts and if information we send does not reach you in due time.

An invoice with amount due (or pro-forma invoice if the payment has been done by credit card through the website) will be faxed within 48 hours after the registration prodecure and also sent by post. In case of payment by bank transfer, it will be mandatory to clearly indicate the number of the invoice on bank transfer order to facilitate the payment identification.
